Responsibilities:
- Deliver comprehensive pharmaceutical care to long-term care residents.
- Monitor and manage patient medication therapies.
- Collaborate with multidisciplinary healthcare teams to enhance patient outcomes.
- Ensure adherence to all regulatory and compliance standards.
- Administer immunizations and participate in health promotion initiatives.
Qualifications:
- PharmD or MTM Certified.
- Immunization Certification.
- CPR Certification.
- In-depth knowledge of long-term care pharmacy practices.
- Excellent communication and interpersonal skills.
