Soliant Health is seeking a skilled Home Health Licensed Practical Nurse (LPN) for a contract assignment in Bangor, Maine. This role offers a meaningful chance to deliver compassionate, hands-on care directly in patients’ homes while supporting their recovery and independence.

Position Details:

• Location: Bangor, ME – 04401
• Schedule: 40 hours per week
• Requirements:
  • Active Maine LPN license
  • Minimum 1 year of home health experience preferred
  • Proficiency with Oasis documentation and Home Care Home Base (HCHB) preferred
  • Current AHA BLS certification

Responsibilities:

• Provide skilled nursing care to home health patients
• Assist with medication management and health education
• Collaborate with RNs and interdisciplinary teams to implement care plans
• Accurately document care delivered using required systems
