Responsibilities:
- Collaborate with the interdisciplinary healthcare team to provide comprehensive pharmaceutical care to oncology patients.
- Review medication orders, ensure appropriate dosing, and monitor for drug interactions and adverse effects.
- Conduct therapeutic drug monitoring and adjust dosages as needed.
- Provide medication counseling and education to patients regarding treatment plans and potential side effects.
- Participate in clinical rounds, tumor boards, and treatment planning meetings.
- Maintain accurate patient records and documentation.

Qualifications:
- Current Pharmacist license in the state of California.
- PharmD degree from an accredited college of pharmacy.
- Minimum of 2 years of clinical pharmacy experience in oncology.
- Strong knowledge of chemotherapy agents, supportive care medications, and oncology treatment guidelines.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a fast-paced, team-oriented environment.
