This 13-week assignment provides an excellent opportunity for a dedicated Licensed Practical Nurse (LPN) eager to explore Augusta’s Southern charm while delivering top-notch home health care. Responsibilities include medication administration, wound care, and educating patients and families on health management.

Key Responsibilities:

• Deliver high-quality home health care services, including administering medications, monitoring vital signs, and providing wound care.
• Collaborate with RNs and other healthcare professionals for comprehensive patient care.
• Educate patients and families on managing health conditions and self-care practices.
• Maintain accurate documentation of patient progress and adjust care plans as necessary.

Requirements:

• Active GA LPN license (or compact license).
• Minimum of 1 year of recent home health experience.
• Strong communication and organizational skills.
• Familiarity with Homecare Homebase preferred.
