Advanced Practice Pharmacist Position – Ambulatory Care - Rheumatology.

OVERVIEW:

• A true Advanced Practice role. 100% clinic-based
• Focus on Rheumatology 
• Will initiate, modify and discontinue medications within a Collaborative Practice Agreement as defined by the California State Board of Pharmacy.
• Focus on management of complex disease states to help patients reach optimal therapeutic goals.
• Simplify patient medication regimens, minimize the risk of harmful medication interactions and educate patients to enhance adherence and clinical outcomes.
• Responsible for policy development, continuous process improvement, continuing education, clinical and medical staff development and Pharmacy APPE student and resident preceptorship.
• Performs patient care assessments, orders and interprets laboratory tests, prescribes, modifies and discontinues medication therapies.
• Monitors clinical outcomes, communicates with medical providers, performs electronic documentation and refers patients to other health care providers.

COMPENSATION:

• $210,000-$225,000 annually
• $10,000 Bonus
